Job Description:
A company is looking for a Virtual Chronic Care Management Nurse
(Contractor).
Key Responsibilities
Conduct virtual assessments and provide ongoing updates on
patients' health goals
Educate patients and families on conditions, treatment options, and
self-management strategies
Document patient interactions and care plans accurately in
electronic health record systems
Required Qualifications
Bachelor's degree in Nursing (BSN) from an accredited institution;
Master's degree (MSN) preferred
Active Registered Nurse (RN) license in good standing
Minimum of 2 years of clinical experience in chronic care
management, preferably for Medicare beneficiaries
Proficiency with Electronic Health Records (EHR) systems
Comfortable with technology and adapting to new virtual care
platforms
