Job Description:
A company is looking for a Care Manager (ICC - Youth) to monitor
and manage mental health and substance abuse treatment
services.
Key Responsibilities
Monitor residential and outpatient care services for medical
necessity and effectiveness
Develop and oversee discharge plans in collaboration with providers
and facilities
Participate in quality improvement activities and advocate for
patient treatment needs
Required Qualifications, Training, and Education
Current licensure as CEAP, LCSW, LMSW, LPC, LSW, or RN
Minimum of 2 years of experience in healthcare, behavioral health,
or substance abuse settings
Knowledge of utilization management procedures and community
resources
Experience in inpatient and/or outpatient settings
Understanding of DSM V or the most current diagnostic edition
