Job Description:
A company is looking for a Care Manager (RN) to develop and
facilitate complex care management activities for members with
physical needs.
Key Responsibilities
Evaluate member needs and recommend care plans to address barriers
to accessing appropriate care
Develop ongoing care plans and collaborate with providers to meet
members' unmet needs
Monitor member status and revise care plans as necessary to achieve
health goals
Required Qualifications
Degree from an Accredited School of Nursing or a Bachelor's degree
in Nursing
2 - 4 years of related experience in care management
RN - Registered Nurse - State Licensure or Compact State Licensure
required
